"Out-of-Network" & Superbill Guide for Ketamine Treatment

Most insurance plans do not directly cover IV Ketamine infusions for mental health because it is an "off-label" use. However, you may be able to get partial reimbursement. If you have "out-of-network" benefits (often found in PPO plans), we can provide you with a Superbill. You submit this itemized receipt to your insurance company, and they may reimburse you for a percentage of the cost depending on your deductible and plan details.
