Insurance Coverage for Spravato & Ketamine in Maryland

Lumin Health Maryland accepts most major insurance plans for Spravato (esketamine) treatment. Because Spravato is FDA-approved for treatment-resistant depression, it is classified as a covered medical treatment — not an elective procedure.

Our insurance team verifies your coverage, obtains prior authorization, and provides a clear cost estimate before your first session. Most patients pay only their standard specialist copay ($10-$40 per session).

Maryland Insurance Coverage for ketamine therapy and esketamine (Spravato)

Lumin Health accepts Medicare and most private insurance plans offered in Maryland, including CareFirst
UnitedHealthcare, Cigna, Kaiser Permanente, Triicare, Elevance, Aetna, and Humana.

Don't see your plan listed? Contact us — we may still be able to work with your insurance. Our team will verify your specific coverage at no charge.

Is Ketamine Covered by Insurance?

Yes, most plans cover Spravato (esketamine), but rarely IV ketamine.

  • Spravato (Nasal Spray): ✓ Covered by Aetna, BCBS, Cigna, Medicare, and more for Treatment-Resistant Depression.
  • IV/IM Ketamine: ✕ Not Covered by most commercial insurers (considered "off-label").
  • Lumin Health: We are in-network with all major commercial payers for Spravato treatment.

How Lumin Health Works with your Insurance to Cover Esketamine Therapy

The Lumin Health team is dedicated to making these transformative treatments as accessible as possible. We have extensive experience working with a wide range of insurance providers and would be grateful to champion your case to verify your coverage, explain your benefits in simple terms, and manage the authorization process on your behalf.

The first step is to understand if your insurance plan covers the cost of ketamine. Our team offers a free and confidential insurance verification to determine your plan's benefits for Spravato (esketamine) treatments.

Step 1

Verification of Benefits

Lumin Health will contact your insurance provider to confirm your eligibility and benefits.

Step 2

Prior Authorization

If required, Lumin Health will handle the submission of all necessary clinical information for prior authorization.

Step 3

Beginning Ketamine Therapy

Lumin Health will provide you with a clear summary of your coverage, including any deductibles, copayments, or out-of-pocket estimates. We then decide together on the best course of action for treatment.
You’ll soon be on your way to feeling better!

Costs
With insurance coverage, appointment costs regularly come out to the cost of a typical office visit. This can have massive impact on accessibility of treatment.
Intramuscular ketamine injections typically are $500/session, but pricing is determined on number of sessions booked.
Administration route
Intranasal spray
Intravenous infusion, intramuscular injection, oral lozenge, or intranasal spray

"Out-of-Network" & Superbill Guide for Ketamine Treatment

Most insurance plans do not directly cover IV Ketamine infusions for mental health because it is an "off-label" use. However, you may be able to get partial reimbursement. If you have "out-of-network" benefits (often found in PPO plans), we can provide you with a Superbill. You submit this itemized receipt to your insurance company, and they may reimburse you for a percentage of the cost depending on your deductible and plan details.
Recommendation: Call the number on the back of your card and ask: "Do I have out-of-network benefits for mental health office visits?"
