Yes, most insurance companies cover the FDA-approved version of ketamine called Spravato (esketamine), a highly effective solution for treating depression. However, insurances do not typically cover intravenous (IV) or intramuscular (IM) ketamine treatment for conditions associated with depression, anxiety, or other mood disorders. Esketamine therapy is a great option for those looking for ketamine therapy or ketamine infusion treatment, and provides very similar benefits for those dealing with mood and mental health issues. It’s generally well-tolerated and convenient due to its nasal spray form.  

Lumin Health accepts Medicare and most private insurance plans offered in Massachusetts, including Aetna, Anthem, Blue Cross Blue Shield (BCBS), Cigna, Evernorth, Harvard Pilgrim, Optum, Point32Health, Mass General Brigham Health Plan, and United Healthcare.

Please note that we are out-of-network for MassHealth so any care we provide would be out-of-pocket. Lumin Health only works with some plans in the Tufts umbrella, and Lumin Health unfortunately cannot work with Health Together/Health Direct/OneCare/ConnectorCare Tufts Health Plans.

"Out-of-Network" & Superbill Guide for Ketamine Treatment

Most insurance plans do not directly cover IV Ketamine infusions for mental health because it is an "off-label" use. However, you may be able to get partial reimbursement. If you have "out-of-network" benefits (often found in PPO plans), we can provide you with a Superbill. You submit this itemized receipt to your insurance company, and they may reimburse you for a percentage of the cost depending on your deductible and plan details.
Recommendation: Call the number on the back of your card and ask: "Do I have out-of-network benefits for mental health office visits?"
